Application and Registration Forms

Here you can find links to the various forms required by prospective, new, and returning students. Although these forms can be accessed from elsewhere on the site we thought it would be convenient to make them accessible from a single page as well.

All downloadable (printable) forms are in Adobe Portable Document Format (PDF) format. If you do not already have Adobe Reader, click on the icon to the right to download it FREE. If you have trouble opening or printing the forms, please contact us and we will mail you a printed copy.

Here are some deadlines:

• For admission to the MFA to begin with the August residency and fall semester, March 15
• For admission to the MFA to begin with the January residency and spring semester, October 1
• For consideration for a scholarship to begin with the August residency, March 15. You must also apply for admission to the program by that deadline
• For consideration for a scholarship to begin with the January residency, October 1. You must also apply for admission to the program by that deadline
• For admission to the residency non-credit option for the August residency, applications are accepted May 15-June 15 on a first-come, first-served basis. The residency option is limited to 10 participants, with no more than 5 in any one genre.
• For admission to the residency non-credit option for the January residency, applications are accepted October 6-November 6 on a first-come, first-served basis. The residency option is limited to 10 participants, with no more than 5 in any one genre.
